Your Story Community Support (YSC) provides a range of supports and services, including:
NDIS Supports: YSC offers a variety of services under the National Disability Insurance Scheme (NDIS) to help individuals with disabilities achieve their goals and improve their quality of life.
Support Coordination: YSC provides support coordination to help NDIS participants navigate the system, connect with service providers, and effectively utilize their NDIS plans.
Supported Independent Living (SIL) and Accommodation: YSC offers SIL services to support individuals in living independently while receiving the care and assistance they need. This includes both shared and individual accommodation options.
Out-of-Home Care (OOHC): YSC provides OOHC services for children and young people who cannot live with their families, ensuring they have a safe and supportive environment.
Access Program: YSC runs an access program to assist individuals in overcoming barriers to services, resources, and opportunities, empowering them to achieve greater independence and community participation.
